Enrolment

We are enrolling patients but only newcomers to the town who live within our Practice Zone.

 

To enrol you can just visit the practice or click the links below to download the three forms and follow the instructions:

 

Enrolment Form – complete a separate form for each member of the family.  Parent or guardian sign for those age 15 or under.

Request for Transfer of Medical Records – complete one form for the whole family but each family member needs to sign. Parent or guardian sign for those age 15 or under.

Medical Questionnaire – complete one form for each family member.  Give us as much information as you can.  Don’t worry if you can’t remember some of the details – we will be checking this form against the information we receive from your last doctor.

 

Post the Request for Transfer to your last doctor.  Then bring the Enrolment Form and Medical Questionnaire to the practice together with ID (Driving Licence or Passport would be ideal).
